12.12 Quality Processes associated with Operations Training
The two governing quality processes for instructors (names as published in the organisation's quality system; use local process titles where they differ):
- Training Disruption Recovery.
- Instructor / Examiner Notification of Training Disruptions.
There are training records / quality forms available via the secure instructor area of the training document library (or equivalent training records system). These forms allow instructors to advise training disruptions (for example failed checks, incomplete or unsatisfactory training, technical issues) and related incomplete training items.
These messages are automatically routed to all management and support staff that need to respond to these problems.
Additional message forms
Additional quality forms are typically available on the secure site, providing the ability to report training problems directly to the concerned management and staff:
- Missing / Damaged Simulator Documentation. Use this form to report any simulator or briefing-room documentation (charts, QRHs, manuals, etc.) that is either missing or damaged. The message is routed to flight operations support (or the local document-control owner) for action, and copied to training managers for oversight and follow-up.
- Training Progress (IMPROVEMENT REQUIRED). Use this form whenever an overall outcome of S- (IMPROVEMENT REQUIRED), or the local equivalent grade, is assigned for a training session or Line Flying Under Supervision (LFUS) sectors. The message is routed directly to training management for appropriate action. It is intended to provide the fleet training manager with a timely alert about a trainee experiencing progress problems during simulator training or LFUS.
Access paths
Where the organisation publishes both internal and external access to the same forms, the secure instructor page typically labels them separately (for example intranet versus internet / remote access). Use whichever path your roster location and network access require; the form content is the same.
Using the forms
Open the applicable form from the secure instructor area, authenticate as required, and complete the fields. All forms include a free-text field for pertinent details of the problem being reported. Submit according to the form's published control (send or submit).
Instructors are requested to report any problems with use of these forms, or any suggestions for improvements, to their fleet training manager or head of flight training.
